The Living Well Community Wellness & Empowerment Service provides a single, community-based route for preventative wellbeing support across Havering. Commissioned by Havering Council and delivered by Tapestry Care UK, Live Well helps partners support residents earlier, more effectively and in a more joined-up way.
The Living Well Community Wellness & Empowerment Service is delivered through Tapestry’s community hubs in Rainham, Harold Hill and Hornchurch, embedding support directly within local neighbourhoods and alongside trusted voluntary and community sector partners.
What is The Living Well Empowerment and Wellness Service?
Live Well is a preventative, strengths-based empowerment service designed to support residents before issues escalate into crisis or statutory intervention.
It provides:
• A single referral route into community-based wellbeing support
• Coordinated, non-duplicative responses across the voluntary sector
• Flexible, proportionate support tailored to individual needs
The Living Well Community Wellness & Empowerment Service complements — rather than replaces — statutory and specialist services by focusing on early help, social determinants of health and community connection.

The Living Well Community Wellness & Empowerment Service supports Havering residents aged 18+ who:
• Do not meet thresholds for statutory services
• Are experiencing low-level or emerging needs
• Would benefit from coordinated community support

What happens after a referral?
1️⃣ Acknowledgement & triage
Referrals are acknowledged promptly and reviewed to ensure suitability.
2️⃣ Initial contact
A Live Well practitioner contacts the resident (or referrer if appropriate) to introduce the service and agree next steps.
3️⃣ Proportionate assessment
A light-touch, strengths-based assessment identifies needs, goals and appropriate support pathways.
4️⃣ Coordinated support
Residents are supported to access relevant community-based services, with Live Well maintaining oversight to ensure continuity and reduce fragmentation.
5️⃣ Feedback & outcomes
Where appropriate and with consent, referrers can receive confirmation of engagement and high-level outcome information.

The Live Well Empowerment Service is here to support residents across Havering to improve their health, wellbeing and independence, with help that is local, friendly and easy to access.
Delivered by Tapestry Care UK through our community hubs in Rainham, Harold Hill and Hornchurch, Live Well focuses on early help and prevention, supporting people before problems become crises.

Who is Live Well for?
Live Well is for Havering residents aged 18+ who would benefit from extra support with everyday challenges that affect wellbeing.

We aim to make access as straightforward as possible through a single referral route, so you don’t have to repeat your story to lots of different services.
What happens after you get in touch?
1️⃣ Initial contact
Once we receive your enquiry or referral, a member of the Live Well team will get in touch, usually by phone, to introduce themselves and explain the next steps.
2️⃣ A friendly conversation
We’ll have a relaxed, supportive conversation to understand:
• What’s going on for you (or your loved one)
• What matters most right now
• What support might be helpful
This is not a test or judgement. There are no right or wrong answers.
3️⃣ Light-touch assessment
We carry out a simple, proportionate assessment focused on strengths, needs and goals. This helps us agree the best way forward together.
You can choose to:
• Speak by phone
• Meet face-to-face at a local community hub
• Involve a family member or trusted person if you wish
4️⃣ Joined-up support
Following the assessment, we’ll help connect you to the right support. This may include:
• Practical advice and guidance
• Support from local voluntary and community organisations
• Community activities or peer support
• Ongoing check-ins if helpful
We stay involved to make sure support feels joined-up and works for you.
